Cancellation Policy Information specific to your conference will be mailed and/or e-mailed to you in a Presenter Information Packet. ¨ PRESENTER FORMS Due to the amount of detail involved in planning conferences, it is critical that deadlines be met. A list of forms that may be included in your Presenter Information Packet is provided below. Refer to your Presenter Information Sheet for conference deadlines. 1. Presenter
Profile: Each presenter must complete
this Profile in order to include correct credentials, professional
affiliations and contact information. You may also request audio-visual
equipment, and make arrangements for conference meals, travel and lodging on
this form. 2.
Presentation Profile: The information provided on
your Presentation Profile includes your presentation title, description and
in some cases your session objectives. The conference office reserves the
right to edit your title and/or description for final publication in the
conference brochure and program. 3. Speaker
Agreement:
The Speaker Agreement must be reviewed and signed by
¨ CONTINUING EDUCATION REQUIREMENTS Kesselman-Jones, Inc. applies for
continuing education units (CEUs) for many of our
conferences. Presenters are typically responsible
for providing the following materials depending on the type of CEUs being offered for a conference. On your Presenter Information Sheet you may
be requested to submit the following items: 1. CV/Resume Most professional licensing
boards require a copy of a presenter’s
current CV/Resume. 2.
Presentation Outline Flowsheet: If your conference Presenter
Information Sheet requests the submission of objectives as a requirement,
please click here for instructions,
a blank form and an example of a completed form. 3.
CME Full Disclosure Form: For conferences applying for continuing
medical education (CME) credit, speakers must disclose financial
arrangement(s) or affiliation with any organization(s) as well as any unapproved
or investigational uses of products or devices directly related to the
subject matter to attendees of all CME programs. This is a requirement of the ACCME and the
AAPF. For more

home page. ¨ HANDOUTS The conference office will copy handouts if they are submitted by the deadline date outlined in your Presenter Information Sheet. Handout Guidelines: 1. Cover page must include presenter’s name and presentation title 2. Maximum of 20 pages per presentation 3. Handouts will be duplicated double-sided 4. PowerPoint slides will be duplicated 6 slides per page 5. Originals
must be E-mailed to Melissa@KessJones.com
or mailed to 6. Faxed handout originals will not be accepted. 7. Presenters are responsible for obtaining written permission to reproduce copyrighted material. Without a copy of written permission, we will not include any copyrighted material as part of your handouts. 8. If you do not submit your handout(s) by the deadline date, you will be responsible for bringing your handout(s) at your expense. ¨
PRESENTATION Presentations must be planned according to the audience identified on your Presenter Information sheet. It is expected that the Presentation Description submitted by the presenter to the conference office and published in the conference brochure/program match the presentation given at the conference. CHECK IN: Presenters must check in at the conference registration desk at least 30 minutes prior to your presentation. AV: Please refer
to your Presentation Profile regarding audiovisual equipment. If you are requesting additional equipment
than that provided by the conference, we will make every reasonable effort to
secure the equipment for you. We will
notify you immediately if the equipment is not available. ROOM SET UP: Most conference breakout sessions are set up classroom style. Keynote sessions are generally banquet rounds. If you need a special room set up, please indicate your requests on your Presentation Profile. We will make an effort to adjust the room set up according to your needs. ¨ CANCELLATION
